Very informative post, but the conflict diagram for the roundabout only shows the 4 conflicts which occur as motorists enter the roundabout. It doesn't show the other four where motorists exit, the most hazardous with the old design. Also, the number of conflicts for any given movement is a subset of the total. A cyclist going to the first exit in the roundabout would encounter no conflicts; the second, two; the third, four. Similarly for the other diagrams.

Also, cyclists may control the lane in the roadway rather than using a path around the outside in countries where that is legal -- or there may be no path. This is faster, reduces space requirements and is preferred and safe for smaller roundabouts and neighborhood traffic circles, and/or skillful cyclists. Example: https://vimeo.com/61988764

Sustainable safety would try to avoid having bikes and trams sharing the same road as much as possible. Most bike paths outside the city center will cross at 90 degrees, but turning left on a grand union will inevitably cause some less favorable angles.
It's possible that in the Netherlands more care is given to a smooth embedding of tracks in the road surface. I know for sure that's done for manhole covers.

What is usually done in such a case, which is much safer, is to build an extra lane on the outside, as this leaves no conflict. This extra lane would only be for that left turn. With the addition of traffic signals and with a larger setup, other lanes on the outside can be constructed as well, but then it's best to provide for cyclists going under or over the roundabout

I underwent a similar learning curve in Toronto. My bike is a Pashley Roadster Sovereign with standard 28" tyres. The closest that I have ever come to crashing since I was six years old was on tram tracks.

Normally I am very careful to take tram tracks at a 90 degree angle, but one day an ambulance came behind me with siren on. So I thought "better get off the road," didn't think to check for tram tracks in the heat of the moment and YIKES!

I thought that with the Dutch principle of Sustainable Safety there would be some technical means of making tram tracks safe.
